At the end of the series, it's House's most important relationship at stake when James Wilson reveals he has inoperable cancer. Despite being an oncologist, he rejects the conventional therapy for his thymoma and instead gets House to agree to try a short course of radical chemotherapy in order to shrink the tumor and make it operable. Wilson survives the treatment, but it doesn't work - the tumor remains inoperable, meaning that without treatment, Wilson will survive no longer than five months and, even with treatment, three years would be an exceptional lifespan. However, it's House soon having woman problems as his immigration wife Dominika returns from Atlantic City to tell House that the immigration authorities are coming to test whether their marriage is real. House tries to blow her off, but an offer of $30,000 if he succeeds is too much for him. Unfortunately, they blow the test, but Dominika pleads that she loves House and asks that he not be punished.
However, when they all fail, he comes up with the craziest scheme yet - faking his own death. This one works and he has the pleasure of texting Wilson during his own funeral to tell him to shut up. The two men head off on motorcycles to enjoy the rest of Wilson's life and House's death.

Meanwhile, Wilson decides on a radical and potentially fatal course of treatment for his cancer, which House agrees to administer himself.
An independent blind man (Michael B. Jordan) is admitted to the hospital after suffering auditory hallucinations, and soon begins coughing up blood. Meanwhile, House’s mother (Diane Baker) surprises him at Princeton Plainsboro to tell him about a new relationship with his old doctor (Billy Connolly), who might be his biological father. A teenage boy (Harrison Thomas) attempting to follow in his deceased father's footsteps suffers paralysis at a party. As the team searches for answers, they discover a family secret that threatens the young boy's treatment.
After failing to get out of his upcoming jail time, House disappears and his team, along with Wilson and Thirteen (Olivia Wilde), are unable to locate him. He awakens in a burning building after shooting heroin with his former patient (James LeGros). With his life crumbling around him, House hallucinates people from his past (Braugher, Dudek, Morrison, Penn, Tamblyn, Ward) as he decides whether his life is worth living anymore.
Although he plans to withdraw from the practice of medicine, it appears the real co-dependent relationship is between House and the hospital. With House looking at serving at least another six months, the new Dean of Medicine, Eric Foreman approaches House with an offer to return to work. However, when treating a chemistry teacher, Chase and Adams are in the room when the patient suffers a psychotic break.
A marriage counselor (Jake Weber) collapses during a speaking engagement, when it is discovered that, in addition to vision and liver problems, his low testosterone levels are affecting his marriage and career for the better. Meanwhile, House's wife returns to prepare House for their meeting with Homeland Security for her green card, as the team vies for a position as House's second-in-command. House and his team face a disciplinary hearing with Dr. Cofield (Jeffrey Wright) after a chemistry teacher (David Anders) is admitted to the hospital with paralysis. But things turn bad when in a fit of psychosis and violence, the patient stabs Chase, ultimately forcing the team into a race against time as they battle to keep Chase alive in the ER.
In the melee, Chase is badly stabbed and the life-saving surgery nearly costs him the use of his legs as well. He takes a lengthy leave of absence and, when he returns, has an inappropriate relationship with a patient. Chase's near-death experience has him questioning if he should return to House's team.
Both his ex-wife and ex-girlfriend are furious with him, and he's on the hook for two child support payments and one alimony payment. In addition, Rachel announces that she wants to move to Seattle with her new boyfriend Phil and take Sophia with them. House gets the money to re-fund his department by pawning a piece of hospital equipment to bet on the market. Chase eagerly returns, and even Taub prefers the excitement of saving lives to the routine of fixing noses. However, House initially tries to reconnect with Thirteen, he finally sends her off with her girlfriend to live in Greece.
Meanwhile, House and Wilson keep betting on what is causing a woman's apparent asexuality and Foreman debates whether or not he should petition House's parole officer to remove his ankle monitor early. The series finale aired on May 21, following a retrospective episode titled "Swan Song". In early 2012, it was confirmed that this season was House's last, and it would have a "swan song" episode (a two hour finale consisting of a retrospective and the series finale). Olivia Wilde, Kal Penn, Anne Dudek, Sela Ward, Amber Tamblyn, and Jennifer Morrison also returned for the finale. A soldier (Arlen Escarpeta) returning from overseas, accused of treason after releasing a video showing soldiers killing civilians, collapses and is taken to Princeton-Plainsboro.
